To successfully register, please follow these two steps:

  1. Complete the Online Pre-Registration Form: click the "New User" option on the left side of this screen. Enter all information and submit your session.  

  2. Gather Required Documents: Prepare and have ready all necessary supporting documents (e.g., birth certificate, proof of residency, physical exam, and immunization records). Instructions for submitting these documents will be included in your confirmation email.

What Happens Next?

After you successfully complete the pre-registration form, you will receive a confirmation email within 1-2 business days. This email will contain:

  • A list of the required documents.

  • Specific instructions for submitting your documents and completing the rest of the registration process.
